Step 4: Forward assessment form to assessors

*Note each applicant is responsible for confirming that the required assessment forms and letters are received by the Program Office.


  • Download, complete and submit the assessment forms to your assessors

Required Documents:

  • Three recommendation letters
  • Three assessment forms
  • You will need to provide your assessors with the following information before they submit the recommendation letter and form:
    • Your name (as entered in the application)
    • Your phone number & your email
  • Assessment forms can be emailed to with the subject line: "2018 Assessment"

Assessors are allowed to forward copies of the recommendation letter and assessment form via-email, but are required to mail in the hard copy version of the recommendation letter.
